Diagnosis: “The manager is great but swamped. She is superb with projects and accounting but not so good with the staff. The staff is acting out, and I am afraid the manager will blow her stack and all will walk out, including the manager.”

Prescription: “I need a gorilla—er, I mean—a personnel manager to get the troops in line.”

Management strategies for osteoporosis and osteopenia
A 60-year-old woman came into our office for her annual examination. We noted that she had lost 1.5 inches in height. We secured an immediate appointment with the dual-energy x-ray absorptiometry (DXA) technician. The result was a T-score of –1.5 (low bone mass, osteopenia).

  • How to treat her condition?
  • Was a prescription therapy needed?
  • Which therapy (eg, bisphosphonate, selective estrogen-receptor modular [SERM], or parathyroid hormone [PTH]) was most appropriate?
  • How should she be treated in light of the many complications from long-term therapy?

Dear Dr. Packer,

By way of background, I am 53 years old. I was once very athletic, and still like to hit a tennis ball, go for a bike ride, walk and hike a bit, and swim. In addition, I write lengthy reports on the computer and read research journals. I send e-mails, use smart phones and like to read. During the Fall and Winter, I commute home after dark. I have never had prescription glasses, though have been using reading glasses the past few years, so I'm still getting used to glasses as a lifestyle.
