Gastroesophageal Reflux Disease (Pediatrics) (American College of Physicians PIER)
Gastroesophageal reflux disease (GERD) is very common in infancy, usually peaks at 4 months of age, and usually resolves between the ages of 12 and 24 months at the latest. In most infants with vomiting, history and physical examination are sufficient to reliably diagnose GERD. Certain conditions and prematurity are associated with GERD.
In diagnosis, be aware of the diagnostic criteria for GERD in infants, and avoid attributing other symptoms to GERD; be aware of the value and limitations of questionnaires to aid in the identification of infants with GERD; consider intraesophageal pH monitoring in a subset of patients who do not respond to conservative management; consider other tests in patients with atypical presentations who may have GERD or other conditions with similar symptoms; and consider other GI diseases in patients with atypical presentations of gastroesophageal reflux.
