Pharmacists can lower readmissions with discharge counseling and housecalls

The need to prevent hospital readmissions (and possible financial penalties) makes pharmacist-patient consults a no-brainer, both before discharge and after patients return home. Contributor Bob Katz has been active on both fronts and has some first-hand tips.

I volunteered to set up a program for a local hospital. The plan was that during discharge, each patient would have the opportunity to discuss medications with a pharmacist. Everyone was excited about this except the medical staff.
